I and my spouse have decided that I should retire from my job at UPS last month. When trying to retire SS requires a form from the employer about insurance coverage periods. I have sent Teamcare a copy of the requested information by fax. Awaiting a response from Teamcare. UPS at my location no longer has a human resource department. I already have been terminated from my job at UPS so I don't know whom to complete my documents. Any suggestions?
