I'm just amazed he made it all of the way to the interstate before that back box let go.

I've seen a FedEx guy drop a rear box right in the middle of I80 in Berkeley CA. The fact that he made it to where he was leaves me to suspect equipment failure. Whether or not it was detectable in a pretrip? Who knows.
