A first shirt/Sgt is supposed to be the highest ranking enlisted man in the unit.
He serves as a liaison between the commander and enlisted men to make the commander more accessible to the enlisted ranks
The term first shirt comes from way back when the highest ranking enlisted man got first pick of new uniforms, or he got the "first shirt" out of the box
