Heavy duty trucks very typically have LH threads on the driver's side wheels. Dodge light-duty pickups used to be that way also.

Regarding the pic, I think I would consider cutting off the lug nut horizontally with an electric die grinder. Also, it may seem counter intuitive but 'tightening' a stubborn fastener slightly, and then loosening it sometimes does the trick.
